Carotid Rete--Edelman NH, Epstein P, Cherniack NS, Fishman AP Cardiovascular-Pulmonary Division, Department of Medicine, University of Pennsylvania, Philadelphia, Pennsylvania 19104 ; --Amer J Physiol 223: 615-619 Sept ; 1972 * A procedure has been developed for continuously measuring cerebral blood flow in the goat. This method takes advantage of the unique anatomy of the cerebral vasculature of this species in order to measure unilateral cerebral blood flow with an electromagnetic flowmeter. Studies using radioactive particles indicated that flow measured in this manner included a small quantity of noncerebral blood flow 5% to 15% ; , primarily that of the eye. Using this procedure, the cerebral vasculature of the goat was shown to dilate during acute hypoxia and hypercapnia in a manner similar to that observed in other mammals. Separate studies were done to characterize the behavior of the vessels that comprise the carotid rete. These indicated that the vessels of the rete constrict in response to norepinephrine, dilate in response to isoproterenol, and are unaffected by an appreciable increase in arterial P r 0 approximately 9 mm Hg ; this respect, they behave like systemic rather than intracerebral vessels. 4. Relator filed an appeal, submitting a new medical report from Charles L. Walters, M.D. However, SERS denied the appeal and upheld the termination of benefits. 5. In August 2000, relator filed a new application for disability benefits. Frederick Stockwell, M.D., signed the attending physician's report on the application, opining that relator had been disabled from operating a school bus since August 1997. 6. In August 2000, Dr. Stockwell provided an additional report, explaining that relator suffered from lumbar degenerative disc disease with sciatica and that she had fibromyositis with recurring exacerbations of pain all over. He reported that relator had chronic persistent depression with exacerbations, that she was easily fatigued, and did not have the stamina to perform her regular duties. Dr. Stockwell described worsening symptoms over the years and noted that, in recent months, her lumbar spine evaluation showed degenerative disc disease. He also noted that relator had "multiple other chronic medical problems" including hypothyroidism, irritable bowel syndrome, and a history of breast cancer currently in remission. Dr. Stockwell stated that relator had positive antinuclear antibody tests going along with her fibromyalgia and arthritic type symptoms, and he concluded that relator could not perform her duties as a school bus driver with sufficient comfort, endurance, and emotional stability. 7. Relator also submitted numerous additional reports: an x-ray report from May 1997 indicating "mild" osteoarthritic changes of the spine with "mild" generalized deossification of the bones; a December 1998 x-ray of the lumbar spine showing "no significant interval change" from the prior x-ray; a December 1998 CT scan showing "mild" disc bulge at L4-5 with questionable effacement to the right L4 nerve root and a.

Christensen * , E. Jrgensen, J. Attermann, P. Sidenius, J.H. Poulsen. * Department of Clinical Pharmacology, Aarhus University Hospital, 8000 Aarhus C, Denmark. Objective. To examine the relationship between serum sodium and the concentration the active metabolite of oxcarbazepine: 10-hydroxycarbazepine MHD ; -. Further, the objective was to examine their association with other possible predictors of hyponatremia: plasma creatinine, age and sex. Design. Cross-sectional study of simultaneous laboratory measurements. Setting. Aarhus University Hospital. Subjects. Records on 606 patients, who had simultaneous measurements of MHD, serum sodium and plasma creatinine between December 1995 and April 2002. Results. Serum sodium was inversely correlated to the plasma concentration of MHD. The association remained in a multiple regression analysis. Serum sodium was inversely correlated to age but positively correlated to serum creatinine. The sodium level was equal in the two sexes. Conclusion. The concentration of MHD correlates with the serum sodium level in patients treated with oxcarbazepine. Age and serum creatinine, but not sex also significantly affects the serum level of sodium. These factors should be taken into consideration when prescribing oxcarbazepine. B.4 NITRIC OXIDE SYNTHASE INHIBITION DOES NOT IMPROVE RENAL FUNCTION IN CIRRHOTIC PATIENS WITH ASCITES H.C. Thiesson1, 2, 3, O. Sktt1, B. Jespersen2, O.B. Schaffalitzky de Muckadell3. 1 Department of Physiology and Pharmacology, U niversity of Southern Denmark, Odense Denmark, Departments of 2Nephrology and 3Gastroenterology, Odense University Hospital, Odense, Denmark. Based mainly on animal experiments, nitric oxide has been proposed to account for the peripheral arterial vasodilation and hyperdynamic circulation in liver cirrhosis. The aim of this study was to clarify whether a reduction of nitric oxide synthesis would ameliorate the circulatory and renal dysfunction in decompensated cirrhotic patients. The effects of NG-monomethyl-L-arginine-acetate, a nitric oxide synthesis inhibitor, were studied. After a 60 min basal period, a total of ten patients received increasing doses of NGmonomethyl-L-arginine-acetate, five patients received 12.5, 25, and 50 g kg min ; and five patients received 25, 50 and 100 g kg min ; as a constant infusion during 3 hours followed by a post infusion period. Five patients received saline infusions only. NG-monomethyl-L-arginine-acetate infusion resulted in an increased blood pressure, decreased heart rate, and dose-dependent suppression of renin of up to 42.1 7.1%, p 0.01 ; and angiotensin II of up 39.9 9.6 %, p 0.01 ; levels. Sodium and water excretion were not improved, most likely because of a reduction in renal blood flow of up to 29.1 8.1 %, p 0.01 ; . Conclusion: Despite a partial correction of the hyperdynamic circulation, inhibition of nitric oxide synthesis does not improve sodium and water excretion in decompensated cirrhosis, probably due to an accompanying decrease in renal plasma flow. Intrarenal NO synthesis may be important for maintaining intrarenal hemodynamics in decompensated cirrhotic patients and pentoxifylline. Thus, antibiotic use under proper medical supervision only accounts for a small fraction of the population. Anticonvulsants: Although the mechanism of action of anticonvulsant drugs in neuropathic pain states remains to be fully defined, they appear to act as nonselective sodium channel blocking agents. A large variety of sodium channels are present in nervous tissue, and some of these are important mediators of nociception, as they are found primarily in unmyelinated fibers and their density increases following nerve injury. While the pharmacodynamic effects of the various anticonvulsant drugs are similar, the pharmacokinetic effects differ significantly. Carbamazepine has important effects as an inducer of hepatic enzymes and may influence the metabolism of other drugs enough to present problems in patients taking more than one drug. Gabapentin and oxcarbazepine, by contrast, are relatively non-significant enzyme inducers, creating fewer drug interactions. Because anticonvulsant drugs may have more problematic side-effect profiles, their use should usually be deferred until antidepressant drugs have failed to relieve pain. i. Gabapentin Neurontin ; A ; B ; C ; ii. Description Structurally related to gamma-aminobutyric acid GABA ; but does not interact with GABA receptors. Indications Neuropathic pain. Relative Contraindications Renal insufficiency. Dosing and Time to Therapeutic Effect Dosage may be increased over several days. Major Side Effects Confusion, sedation. Drug Interactions Oral contraceptives, cimetidine, antacids. Recommended Laboratory Monitoring Renal function. The parish of Kilglass in Roosky, Co Roscommon became the first Heartsafe Village in Ireland in July 2006. A total of 60 community members in Kilglass were trained in how to perform CPR and how to use an automated external defibrillator AED ; . The organising committee has placed seven AEDs around the village at strategic locations, including the doctor's surgery, the school, the church and other sites easily accessible to the community. The committee has an ongoing plan for skills renewal for its responders and also plans to introduce further AEDs into the surrounding localities of the parish. The Irish Heart Foundation established the Heartsafe Community Programme in March 2005 to help improve survival rates from sudden cardiac arrests in the community. Prescription Drugs under the Mental Nervous Provision. If a plan provides benefits for the outpatient treatment of mental nervous conditions, the examiner should pay for the cost of prescription drugs used in that treatment at the same rate for which physician's visits are paid. This applies to both standard and state-mandated outpatient benefits. When charges are submitted for more than one condition with one diagnosis in the mental nervous category, it is not always cost effective for an examiner to check every prescription number to determine if it is related to the mental nervous diagnosis. In order to expedite minor routine claims where this situation arises, the following guidelines have been developed to assist the examiner in determining whether to pay at 50% mental nervous ; 80% or 100% nonmental nervous ; : If a covered person has other ongoing conditions, which in themselves require medication, and an examiner receives a few prescriptions that are not identifiable, then it is acceptable to pay the charge under one of the nonmental nervous conditions. If the prescription contains either the brand or generic name of the drug, the examiner should use the Physician's Desk Reference book to determine which drugs are used exclusively for mental nervous conditions. If a particular medication, which has been identified, can be used for multiple conditions one of which is mental nervous ; and if the covered person has an ongoing claim for one of the other conditions, then it is acceptable for the examiner to pay the charge under the non mental nervous condition. Contraceptive Drugs. Contraceptive drugs, used as a medication such as in dysfunctional uterine bleeding ; , are covered in the absence of a specific exclusion. However, when these drugs are used for contraceptive purposes they are not covered since they are not being used for the treatment of an illness or injury. Insulin, Needles, Clinitest and Alcohol Swabs. Insulin, needles in conjunction with diagnosis of diabetes ; and clinitests will be considered a covered expense, and will be paid as prescription drugs. However, alcohol swabs will not be considered a covered expense, and should be denied as a nonprescription drug. Vitamins, Minerals, Food Supplements, etc. Vitamins, minerals and food supplements are generally nonprescription are not considered to be covered charges. Food supplements and vitamin compounds which can be obtained by prescription only and are medically necessary will be considered on a case-by-case basis. Antigens. These are protein substances which are introduced into the body to stimulate the production of antibodies. These are not prescription drugs. They should be treated as physician services. Vaccines. Immunizations are normally not covered expenses. Where the administration of pneumococcal or Hepatitis B vaccine is medically necessary as a treatment of a disease, as opposed to preventive care, such may be paid as physician services. | ||
